Please submit the Application Forms to Damla TURGUT, Secretary of the Ethics Committee, both in printed and electronic media (Usb-Memory) separately. Please fill in the forms completely and carefully. The finalisation of the applications may be delayed due to forms filled out without due diligence. Things to Know Before Applying to the Ethics Committee

Please read the explanations in the appropriate tabs below before applying.
It covers studies conducted on humans to reveal or confirm the clinical, pharmacological or other pharmacodynamic effects of one or more investigational products; to identify adverse events or reactions; to determine absorption, distribution, metabolism and excretion; to investigate safety and efficacy.
It covers a new surgical method research, stem cell transplantation research, tissue transplantation research or organ transplantation research to be carried out on humans other than drugs, biological and medical products, medical devices and cosmetic products.
The “Clinical Research Ethics Committee Application Form” must be completed for the above-mentioned researches.
a) Observational studies other than observational drug and observational medical device studies,
b) Survey studies,
c) Retrospective archive searches such as file and image records,
d) Research to be conducted with biochemistry, microbiology, pathology and radiology collection materials such as blood, urine, tissue, radiological images or materials obtained during routine examination, examination, analysis and treatment procedures,
e) Cell or tissue culture studies,
f) Researches to be conducted with genetic material for identification purposes other than gene therapy clinical trials,
g) Research to be conducted within the boundaries of nursing activities,
h) Dietary studies with food additives,
ı) Research on body physiology such as exercise,
i) Studies based on anthropometric measurements,
j) It covers all researches that do not require the direct intervention of a physician, such as researches on the evaluation of living habits.
For Non-Interventional Clinical Research, the “Ethics Committee Application Form for Non-Interventional Clinical Research” must be completed.

3. A photocopy of the relevant departmental board decision of our research assistants who will apply for thesis (Mandatory for Thesis Studies)
4. CV Form of the Applicant (Mandatory)

6. Sample Informed Voluntary Consent Form (Study dependent)
7. Biological Material Transfer Form for Multicentre Research
8. Blue Coloured Plastic File with Transparent Cover (Mandatory)
9. 3 pieces of literature on the subject in Acrobat Reader (.pdf) format: Literature must be in electronic format (Mandatory)
10. If the studies and data you will use in your research belong to different departments/branches, you must fill out form 12 in the Application Forms.
Note
1. Biological material transfer form will not be filled in single-centre studies.
2. The ethics committee application of our Research Assistants who do not have a chair board decision will not be evaluated.
